Subacute thyroiditis triphasic clinical course

The triphasic clinical course of subacute thyroiditis:
Subacute thyroiditis usually presents initially with signs of hyperthyroidism as large amounts of thyroid hormones are released from the destroyed glandular tissue. Then, as storage runs out, thyroid hormone levels begin to drop and thyroid-stimulating hormone (TSH) rises to compensate. Eventually, months after the onset of the disease, hormonal levels stabilize into a euthyroid state.

  • Ultrasonography:
    • Infectious thyroiditis presents with abscesses.
    • Subacute thyroiditis presents with diffuse heterogeneity and low vascular flow Flow Blood flows through the heart, arteries, capillaries, and veins in a closed, continuous circuit. Flow is the movement of volume per unit of time. Flow is affected by the pressure gradient and the resistance fluid encounters between 2 points.   • Fine-needle aspiration:
    • Infectious thyroiditis shows fluid collection with microbes.
    • Subacute thyroiditis shows multinucleated giant cell Multinucleated giant cell Multinucleated masses produced by the fusion of many cells; often associated with viral infections. In aids, they are induced when the envelope glycoprotein of the HIV virus binds to the CD4 antigen of uninfected neighboring T4 cells. The resulting syncytium leads to cell death and thus may account for the cytopathic effect of the virus. Erythema Nodosum granulomas Granulomas A relatively small nodular inflammatory lesion containing grouped mononuclear phagocytes, caused by infectious and noninfectious agents. Sarcoidosis.

Management

The type of therapy used will depend on which phase the patient presents or the specific cause of the condition.

  • Thyrotoxicosis Thyrotoxicosis A hypermetabolic syndrome caused by excess thyroid hormones which may come from endogenous or exogenous sources. The endogenous source of hormone may be thyroid hyperplasia; thyroid neoplasms; or hormone-producing extrathyroidal tissue. Thyrotoxicosis is characterized by nervousness; tachycardia; fatigue; weight loss; heat intolerance; and excessive sweating. Thyrotoxicosis and Hyperthyroidism phase: antithyroid medication should not be used
    • Nonspecific beta-blockers Beta-blockers Drugs that bind to but do not activate beta-adrenergic receptors thereby blocking the actions of beta-adrenergic agonists. Adrenergic beta-antagonists are used for treatment of hypertension, cardiac arrhythmias, angina pectoris, glaucoma, migraine headaches, and anxiety. Class 2 Antiarrhythmic Drugs (Beta Blockers) block peripheral conversion of T4 to T3.
    • NSAIDs NSAIDS Primary vs Secondary Headaches in the case of painful goiters (e.g., infectious thyroiditis)
  • Hypothyroid phase: lifelong oral administration of L-thyroxine L-Thyroxine Thyroid Replacement Therapy
  • Riedel’s thyroiditis may require prednisolone Prednisolone A glucocorticoid with the general properties of the corticosteroids. It is the drug of choice for all conditions in which routine systemic corticosteroid therapy is indicated, except adrenal deficiency states. Differential Diagnosis

The differential diagnosis of thyroiditis depends on the differentiation/recognition of the various conditions that can produce inflammation Inflammation Inflammation is a complex set of responses to infection and injury involving leukocytes as the principal cellular mediators in the body’s defense against pathogenic organisms. Inflammation is also seen as a response to tissue injury in the process of wound healing. The 5 cardinal signs of inflammation are pain, heat, redness, swelling, and loss of function. The thyroid gland is a highly vascular, brownish-red gland located in the visceral compartment of the anterior region of the neck. Thyroid Gland: Anatomy gland. The following are the most common causes and their characteristic presentations.

  • Subacute thyroiditis (De Quervain’s thyroiditis): Patients Patients Individuals participating in the health care system for the purpose of receiving therapeutic, diagnostic, or preventive procedures. Clinician–Patient Relationship present with hyperthyroidism Hyperthyroidism Hypersecretion of thyroid hormones from the thyroid gland. Elevated levels of thyroid hormones increase basal metabolic rate. During the hyperthyroid phase, the patient presents with neck Neck The part of a human or animal body connecting the head to the rest of the body. Peritonsillar Abscess pain Pain An unpleasant sensation induced by noxious stimuli which are detected by nerve endings of nociceptive neurons. Pain: Types and Pathways, a tender diffuse goiter Diffuse goiter Smooth and symmetrically enlarged thyroid gland. Goiter, and elevated levels of T3 and/or T4.
  • Acute infectious thyroiditis with abscess Abscess Accumulation of purulent material in tissues, organs, or circumscribed spaces, usually associated with signs of infection. Chronic Granulomatous Disease formation is caused by gram-positive Gram-Positive Penicillins or gram-negative organisms, usually in immunocompromised immunocompromised A human or animal whose immunologic mechanism is deficient because of an immunodeficiency disorder or other disease or as the result of the administration of immunosuppressive drugs or radiation. Gastroenteritis patients Patients Individuals participating in the health care system for the purpose of receiving therapeutic, diagnostic, or preventive procedures. Clinician–Patient Relationship. The organisms most commonly involved are staphylococci and streptococci. Fever is commonly associated with chills, rigors, sweating, and flushing of the skin. Fever and chills Chills The sudden sensation of being cold. It may be accompanied by shivering. Fever. Symptoms can range from a complete inability to swallow, to the sensation of solids or liquids becoming “stuck.” Dysphagia is classified as either oropharyngeal or esophageal, with esophageal dysphagia having 2 sub-types: functional and mechanical. Dysphagia or hoarseness Hoarseness An unnaturally deep or rough quality of voice. Parapharyngeal Abscess and a diffuse, asymmetric goiter Goiter A goiter is a chronic enlargement of the thyroid gland due to nonneoplastic growth occurring in the setting of hypothyroidism, hyperthyroidism, or euthyroidism. Morphologically, thyroid enlargement can be diffuse (smooth consistency) or nodular (uninodular or multinodular). Goiter that is very hard, fixed, and often not clearly defined: Most patients Patients Individuals participating in the health care system for the purpose of receiving therapeutic, diagnostic, or preventive procedures. Clinician–Patient Relationship with Riedel’s thyroiditis are euthyroid, and only a few are hypothyroid. Serum antithyroid antibody concentrations are often high in these patients Patients Individuals participating in the health care system for the purpose of receiving therapeutic, diagnostic, or preventive procedures.
